Fingerprint Background Check Directions

EMT Certification and Recertification Applicants

All EMT certification and recertification applicants must complete a Department of Justice (DOJ) and FBI fingerprint background check through the Live Scan process. Background check results cannot be shared between agencies, so the process must be completed for each agency that requires one.

Step 1: Get the Live Scan form

Download the Request for Live Scan Service form (DOJ form BCIA 8016):

Print three copies: one for the Live Scan agency, one to submit to MCEMSA with your application, and one for your records.

Step 2: Fill out the form

Copy the agency information below onto the form exactly as shown:

Do not complete any other fields on the form. If the form is filled out incorrectly, DOJ may reject it, and the background check will have to be repeated, including paying the fees again.

Step 3: Get fingerprinted

  • Complete the Live Scan within 30 days before the date you plan to apply for certification or recertification.
  • Find a Live Scan site anywhere in California on the DOJ locations page: www.oag.ca.gov/fingerprints/locations.
  • Call the site before you go, even if it does not require appointments, to confirm hours, payment methods, and current requirements.
  • Bring a photo ID.

What it costs

You pay the Live Scan site directly when you submit your fingerprints. The total is made up of three parts: a $32.00 DOJ fee, a $19.00 FBI fee (both non-refundable), and the site's own "rolling fee," which varies by location. Payment methods also vary by site (personal check, money order, cash, or credit card), so confirm before you go.

Step 4: After your Live Scan

Wait 24 hours after your Live Scan, then call the office at (209) 529-5085 to confirm MCEMSA has received your results before submitting your certification application. Results can take up to 30 days.

Certification is issued after review and approval of your application and documentation, the results of the background check, and receipt of the EMT certification or recertification fee.
