Read this before you apply
  • Initial MICN authorization cannot be completed online. It requires regional exams, evaluated radio calls, and an on-site photo. See Initial Authorization below.
  • Re-authorization (renewal) is online only. The office is closed to the public; no walk-ins or appointments.
  • Applications are not accepted by fax or mail.
  • Have every requirement ready before you start. Applications will not be processed until all requirements are met.
  • Applications submitted during the last seven calendar days of the month may be delayed due to Agency workflow.

Initial MICN Authorization

Download the MICN application and complete all of the following steps to become authorized as a Mobile Intensive Care Nurse (MICN) in the counties of Amador, Calaveras, and Mariposa:

  • Have a minimum of one (1) year of experience as a Registered Nurse, including six months of experience in critical care or emergency department nursing.
  • Submit a completed and signed pdf MICN application(288 KB) .
  • Submit your MICN course completion certificate.
  • Submit a current printout from BreEZe (the California Board of Registered Nursing license lookup) showing current license status.
  • Submit a copy of your current ACLS card (front and back).
  • Submit a copy of photo identification (for example, a California driver's license).
  • Submit the 4 MCI Field Operations Module CE certificates (available at www.mvemsa.org), per Agency Policy 853.00, Prehospital Training Standards. These are required every two years.
  • Pay the $100.00 non-refundable application fee.
  • Take and pass the Regional MICN Authorization Exam with a minimum score of 75%.
  • Take and pass the Regional ALS Treatment Protocol Exam with a minimum score of 80%.
  • Satisfactorily complete 10 ALS radio calls, documented on the Regional ALS Radio Call Evaluation Form and evaluated by an authorized Base MICN or Base Physician working in a Base Hospital emergency department. You may not begin your 10 ALS calls until you have passed both regional exams.
  • Have a digital photo taken on site by Agency staff.

MICN Re-authorization (Renewal)

To renew, submit a completed and signed pdf application(288 KB) (hard copy) or use the online application button below, including copies of:

  • A current printout from BreEZe showing current license information.
  • A copy of photo identification (for example, a California driver's license).
  • Proof of successful completion of 16 hours of continuing education related to ALS care, obtained during the current authorization period. CE must be documented by CE certificates (a list of courses and hours is not acceptable), and each course must be at least one hour; shorter courses are not accepted.
  • The 4 MCI Field Operations Module CE certificates (www.mvemsa.org), dated no earlier than 90 days before re-authorization, per Agency Policy 853.00. These 4 hours count toward your 16-hour CE requirement and are required every two years.
  • A current passport-style photo: an actual passport photo, or a clear headshot against a plain background.
  • Payment of the $50.00 non-refundable application fee (cash, check, money order, cashier's check, debit, or credit; PayPal card payments add a $5.00 processing fee).
If your authorization expires. A $15 late fee is assessed for authorizations processed after the expiration date, and additional CE and documentation are required. See Policy 261.00, MICN Authorization/Re-Authorization, for all additional requirements.
How online processing works
  • Gather all required documents, including your passport-style photo, saved and ready to upload before you start the application.
  • Allow 5 to 7 business days for processing and issuance of your MICN re-authorization card once all requirements are met. No exceptions.
  • Incomplete applications are held for 7 days to allow you to submit missing items, then considered abandoned.
  • All fees are non-refundable. Incomplete applications that expire during processing are subject to the $15.00 late fee.
